Join a mission-driven team in the Program of All-Inclusive Care for the Elderly (PACE), providing comprehensive, community-based care to vulnerable seniors. As a Primary Care Physician and Pod Leader, you’ll cultivate long-term relationships with a smaller patient panel—typically eight patients per day—allowing you to see the real outcomes of your holistic, value-based approach within a collaborative interdisciplinary environment.

Key Responsibilities

• Perform detailed in-person assessments and collaborate with the interdisciplinary team (IDT) to create personalized care plans for each participant
• Lead pod operations, including coordination of participant discharges for acute, short-term, and long-term placements
• Provide exclusively outpatient and home-based care—no hospital rounds required—ensuring a focus on comprehensive, patient-centered medicine
• Collaborate with specialists and support staff to deliver coordinated, holistic care

Work-Life Balance

• Standard schedule: Monday–Friday, 8:00 AM–5:00 PM, including scheduled home visits
• Limited rotating on-call responsibilities—phone-based only, a few nights or weekends per month

Qualifications

• Board-certified (preferred) or board-eligible Primary Care Physician in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics
• Eligible for enrollment to serve Medicare and Medi-Cal participants
• Strong passion for geriatric care and interdisciplinary collaboration
